The OSCAR Project forms part of our Post Covid-19 Response, resilience Plan of Organisation systems development and strengthening  to improve business development, efficiency and sustainability of MOD Public Health Foundation (MODPHF) and Public Health Steering Team Uganda (PHSTU).

 

1.0       The Objective: To establish and sustain resource mobilisation and management mechanisms addressing Post COVID-19 issues  and climate change counter measures at MOD Public Health Foundation (MODPHF) and Public Health Steering Team Uganda (PHSTU) by 2024.

1.1    Background

MOD Public Health Foundation is a Non-Government Organisation (NGO) and health promotion development and Implementation Organisation. The Organisation forms part of the Public Health Steering Team Uganda (PHSTU) which was established in June 2023. PHSTU is a Fiscal Agency with the vision to consolidate the collaboration of over 10,000 CBOs, CSOs, NGOs, Local Government, and Enterprises to center Public Health principles at the core of all activities and essentials as catalysts leveraging better quality of life for people. To improve robustness, we use an adaptive public health framework as a mental model to empower local communities to develop themes and solutions that prevent diseases, prolong life, and promote health including engaging in activities that are smart-climate oriented and mindful of eco-conservation to protect the planet. Our approaches emphasize localization while plugging into global aspirations.

 

1.2       Introduction

We borrow heavily from the SGS: the life course perspectives; theories from pathogenesis and salutogenesis; and other sources highlighting the connection experiences of quality life outcomes have with the Behaviour, Environmental, Biological, Social, Economic, Cultural, Political, Medical, Gender, Physical and Civic (Be-Ci) enabling and responsive context. We hope to work with existing NGOs, CBOS, Local Governments, and other entities to bring to the attention of stakeholders where we need to stress interventions that leverage better life outcomes for people in the greater Masaka region and Uganda at large. Our point of analysis is the household which we frame as the enduring life space and life expectancy improvement environment. We see more and more people choosing to live in the peripheral communities, we are seeing grandparents staying active, stronger, and living longer. There are now more people aging with HIV in peripheral communities which means that we need to address risk factors that come with aging and longevity which we are establishing climate change countermeasures. However, In Uganda, 16 people continue to die due to malaria every day, and 30 die due to TB.  Therefore, we use both MOD Public Health Foundation and PHSTU as platforms to promote household-level wellness and quality of life.

 

These are some of the questions guiding our approaches. 

1)      How do we leverage the prolonging of life principle/imperative?

      Use the numbers of households as spaces for promoting a life of members disaggregate members according to age, sex, gender, social, economic, cultural, biomedical, structural, physical, environmental/biodiversity and civil needs and life skills milestones to gauge needs and subsequent interventions based on the SDGs/people/planet/productivity scale.

2)      How do we leverage the prevention principle/imperative?

      Use the numbers of households as a benchmark for figures to reach!

      Use numbers of household members as eligible participants in our interventions

3)      How do we leverage the promotion of health principles/imperative?

      Use the numbers of households and benchmark model homes' character and note deviations from a standard model home or community.

      use household members to champion model home aspirations connect/linkage with Public --Private Mix (PPM) to strengthen supply chain and referral systems and structures.

 

1.3       Vision

We want to use this concept in two ways; to mobilize resources for strengthening the enabling environment, systems and structures, capacity building, training, research, and programme implementation at MOD Public Health Foundation (NGO) and the eleven (11) founder CBOs of Public Health Steering Team Uganda (PHSTU) by centering adaptive public health principles in public health project design and implementation. Secondly, we believe that our model will contribute solutions that can be used in other parts of Uganda for CSOs, and NGOs to embrace the localization agenda to promote interventions that leverage the life outcome for people in the Greater Masaka Region.

 

1.4       Problem Statement


Like most CBOs and CSOs in Uganda, The emergency of COVID-19 in 2020 severely affected MOD Public Health Foundation funding streams for all our community programs. For instance, when our partnership with Mildmay Uganda on the Implementation of the DREAMS Program for Adolescent Girls and Young Women (AGYW) in Mubende Region ended in March 2020, the Organisation began to lose influence in the public health space in all our catchment area. We found ourselves failing to adapt to the COVID situation. We lost all our competent staff and all systems structures for program implementation, monitoring consequently became weak.  This is because our programming was based on short-term projects conditioned by funding from Mildmay Uganda, and Rakai Health Sciences Program which were also being funded by PEPFAR through the Center for Disease Control and Prevention (CDC). For instance, before COVID-19 emerged in Uganda The Organisation had gained vast experience in community organizing, providing training to health facility workers, and Village Health Teams (VHTs) from hard-to-reach rural areas. We pioneered and rolled out such principles as Communities of Practice (COP); Community Systems Strengthening (CSS); Communities of Practice (COP); and Public-Private Mix (PPM) to empower communities to establish themselves as resilient safe spaces. We even integrated concepts of climate-smart action plans into VMMC and SRHR services in Masaka and Mubende Region. However, for three years without funding, we saw these innovations diminishing in communities,  and districts, We wrote over 25 proposals and none of them reached the final stages of approval.

 

1.5       Innovation Robustness


Our innovation follows a visit by Prof. Tom Muyunga Mukasa, A global health specialist from the University of Harvard who came to MOD Public Health Foundation last year in September 2022. Tom introduced to us the concept of Systems and Design thinking to develop qualitative and quantitative organization stories rooted in resilience out of the grassroots communities whose outcomes can be used to inform development work, and HIV/TB/Malaria prevention and regularly engage in strengthening the capacity of local and rural-based CBOs in Uganda to own knowledge and skills that keep the traction toward the 2030 SDGs. We learned to anchor our narratives in Global Aspirations and formed 12 themes from SDGS, introduced to us Global Aspiration and New Funding Model4 frameworks. Tom conducted a three-month master class on writing skills with MOD staff in Brief Writing, Opinion Writing, Concept Writing, Inception Note Writing, Terms of Reference, Interviews, storytelling, and conducting conversations. By applying the 3Ps of Passion, Professionalism, and Productivity, we learned how to mainstream the principles of promoting, protecting, and prolonging life in HIV/TB/Malaria eradication mechanisms for UNAIDS 2030 goals; Climate smart Action Plans, integrating COVID-19 response, recovery, and resilience plans; Maternal and Child Health, Sexual and Reproductive Health Rights (SRHR), and Voluntary Male Medical Circumcision (VMMC). Because of this recalibration, We have written over 60 stories from our previous project implementations, and all posted on various Google blogs, written concept notes on various themes such SRRH, Women and Girls Growing the Gloves-Skills Trainings and Home Management, Climate Smart Action Plans, CSOs Advocacy Movement, Post Covid-19 Response and Recovery Plan for TB/HIV/Malaria by 2030, Health Insurance e.tc. On 24 June 2023, Twelve (12) CBOs endorsed the formation of The Public Health Steering Team Uganda (PSHTU) as a Fiscal Agency and an advocacy platform focusing on centering public health principles in the mission of over 10,000 CBOs/COS/NGOs that will work with us as well those intending to join us from the globe. The idea is to support systems and structures strengthening the needs or gaps of these organizations to catalyze better performance and promote better life outcomes for program participants.

Lastly, we are currently using boda-boda or car hires to implement and coordinate all activities of the MOD Public Health Foundation and those of the Fiscal Agency PHSTU. This increases the cost of doing business, yet we are already constrained.

 

1.6       The Need

As part of the Post Covid-19 Response, Resilience, Sustainability Plan, MOD Public Health Foundation and its affiliation with The PHSTU we have invested substantial efforts to improve systems and structures including documentation through narrative writing, storytelling to reposition the organization in the rapidly changing donor priorities for resource mobilization strengthening strategies and implementation of joint public health projects. Despite the current lack of funding, the innovations through our narratives of communication are improving the organization's visibility and starting to attract both national and international viewers for potential partnerships. For example, we have signed an MOU/Charter with The Western Institute of Public Health for Joint Proposal Writing, Research, and Capacity Building, Our published work has been viewed by organizations such as Amplify Changes, The Global Finance Facility (GFF), University College Cork (UCC) Ireland, Global Fund, Infections Disease Institute, UVRI/MRC, and many other Local CSOs, NGOs, Executive Officers, and Programme managers coming to our offices for learning exposure visits. Therefore, to reposition the MOD Public Health Foundation and the PHSTU, in these unfolding opportunities, we urgently need to have their internal systems, processes, structures, and controls including impact branding and visibility improved and strengthened to enhance strategic partnerships. We need a field vehicle to improve project implementation, coordination, monitoring, visibility and reduce risk to staff, and managers from occupation road risks.

 

2.0       Theme: Organization Development, Strengthening, Capacity Building, Consolidation, Resilience Systems and Structures

 

When funding for systems development is obtained, MOD will ensure effective coordination, system management, and channeling of resources to priorities of the PHSTU founding member CBOs and NGOs including focusing on effective training and management of Stakeholder Population Community Structures (SPCS) at all zones and providing mid-level managers with support, clear guidelines, checklist for program management, monitoring and learning.

 

2.1       Key Urgently Need Activities

Activity 2.1.1: Establish partnership, resource mobilization, and fundraising department at MOD Public Health Foundation including developing a resource mobilization strategy ($19,786)

Activity  2.1.2: Strengthen digital marketing strategy and impact branding; develop website, and Social media Presence (Campaigns) ($2,560)

Activity  2.1.3:  Develop MOD Public Health Foundation Strategic Plan 2023-2030 including relevant tools and Protocols ($2,400)

Activity  2.1.4: Strengthen financial management systems (Finance Management software, recording keeping, and developing/reviewing relevant policies) ($3,733)

Activity  2.1.5: Establish Knowledge Hub at Secretariat for Promoting Research, Climate, Health And Pandemic Preparedness Skills ($6,000)

Activity  2.1.6: Strengthen board capacity and management in cooperate governance (Develop Board Charter) ($533)

Activity 2.1.7:  Operationalize The Public Health Steering Team Uganda (PHSTU) Activities ($2,480)

Activity 2.1.8: Procure a field motorcycle and vehicle to facilitate coordination of PHSTU activities and program implementation ($29,533)

Activity 2.1.9: Strengthen Systems for  Programme Monitoring and Evaluation ($3,013)

2.2       Timelines

We plan to set up the required organizational systems and structures between 2023-2024. The plan will include building Organisation safety nets led by C-Suite Executive Members of MODPHF and PHSTU, and development partners to a culture of systems thinking and development for the period 2023-2030.

 

3.0       Milestones

1.      Available and designated Office space for key departments: Finance and Administration,      Reception unit, Operations, monitoring and evaluation.

2.      Programme Concepts and Inception Notes in Place

3.      Desktop computers are available at the central MOD Public Health Foundation

4.      All PHSTU Organizations have a signed MOU with the Fiscal Agency.

5.      Functional Skilled Board and Management

 

4.0       Cost of Doing Business

The Board of Directors has continued to contribute funds to ensure functionality and the continued presence of the organization and to establish basic systems and structures over the years. We shall continue to mobilize resources widely among the local and international organizations, cooperate organizations, development partners, and friends of MOD Public Health Foundation to improve the effectiveness and efficiency of both organizations.

 

5.0       Business Unusual

We intend to use our Fiscal Agency The Public Health Steering Team Uganda (PHSTU) as a catalyst for resource mobilization, network building, and advocacy. At the same time, we shall build on the capacity of all member CBOs/CSOs/NGOs to advance public health principles for better performance.

 

6.0       Sustainability

The following are strategies that will enhance sustainability.

1.      Strengthened organization systems, structures, and processes will enhance accountability and operational efficiency. This will enhance learning, engagement for business development, resource mobilization, and further partnership development.

2.      Establish a centrally placed knowledge hub to act as a drop-in resource center for member CBO/CSO/NGO.

3.      Working within/with the development network to share resources in the form of communities of practice (COP), Community Monitoring (CLM), and District Local Government for reporting, technical support supervision, learning, and joint resource mobilization.

 

7.0       Expertise and Operational Capacity

MOD Public Health Foundation and The Public Health Steering Team-Uganda (PHSTU) is made up of members with accumulated experiences in Medicine, Clinical care, Public Health, Epidemiology, Research, Social Development, Social work, Education, parenting, Social-mobilisation, Knowledge Hub management, Incubator management, Accelerator support work, Hack practitioners, Innovator-Hub management, Development Catalysts and connections with the Central Government to link communities to Government programs. We are made up of an executive team, boards of directors, and trustees who are experienced in a variety of transformative skilling. Through a combination of networking and volunteering, we continue to advocate and campaign for supporting grassroots CBOs/CSOs to center public health principles in their programming to promote better health outcomes. We hope that if you provide us with this funding, we shall be able our organization systems, operation structures, and environment to catalyze innovation, resource mobilization, and efficiency. The developed systems will be able to attract strategic partnerships that will synergize the development of public health projects that link with program participants to government programs such as Emyooga, Parish Development Model (PDM) Water, Sanitation and Hygiene projects (WASH), Climatic Smart Actions (CSA), Health Promotion education campaigns. All our innovation projects will have a direct connection to the local government including Sub-county chiefs, Health inspectors, Parish chiefs, Traditional and Local leaders Population-Based Structures for sustainability.
